Service requests and proposals
Submitting a form is a request for contact, not a confirmed appointment, purchase or agreement to perform work. Service availability, the responsible provider, scope, price, permits, qualifications, timing and any warranties must be confirmed in a separate proposal or agreement. The energy worksheet is illustrative and does not establish equipment suitability.
